Added UserStuff::EnsureSessionOpen.  Ensure that either there is already a
current session, or we make a new (empty) one.
Remove the $session argument from SetSession.

Log Message:
If ?mbt=1 is specified, set mbt=1 in the session.  Create an anonymous session
if necessary.
Since the tagger first requests /tagger/intro.html?mbt=1, this should mean
that basically all tagger traffic has a session, and has mbt=1 set.
