On the latter first, I've had discussions a few times with some of the
classical editors on it.  Some seem to prefer the "Violin concerto"
formulation, but I'm not one who does - it leads to a situation where we
sometimes have one form, sometimes the other ("Concerto for Orchestra"
immediately comes to mind).  It's also not the format we use for other
forms, such as symphony, etc - my personal preference at least then is to
always use the CSG track title style, rather than inverting "form" and
"instrumentation" for some types of work.

On the former question, you could have different work numberings, depending
on the composer and the catalogue.  Some catalogues just counted all
concertos without regard to instrumentation.  So you have

Concerto No. 1 for Piano...
Concerto No. 2 for Piano...
Concerto No. 3 for Violin...
Concerto No. 4 for Oboe...
Concerto No. 5 for Oboe...

Other catalogues for other composers count each by instrument type, thus:

Concerto for Piano No. 1 ...
Concerto for Piano No. 2 ...

Concerto for Violin No. 1 ...

Concerto for Oboe No. 1...
Concerto for Oboe No. 2...

So that tool tries to allow you to do either form.  :)
