FWIW, I had a period a couple of weeks ago where I kept making
apparently random no votes on perfectly good edits.  It took me a few
days to figure out why, turns out it was really simple.

Several browsers (I daresay most), allow the 'down' key to scroll down
the page. Firefox 3 beta does not, if one radio button is selected,
hitting 'down' selects the next one.  And I'd just taken to using FF
on MusicBrainz for the nice

I know I annoyed the heck out of the people I hit with this, although
I think I've apologised to them all by now :)

The best thing to do is write a polite edit note with a 'Did you
forget to post a note on this no vote?' kind of question.
