[Playlist] copyright assignment

Hey folks,

Xiph's pro bono counsel came up with this document for assigning our 
copyright interest to Xiph.org:

The idea is for folks who have any potential copyright claim to fill out 
the form and fax it over to Xiph.  People who are named as having a 
copyright interest are: Dan Brickley, Dave Brown, Matthias Friedrich, 
Lucas Gonze, Kevin Marks, Robert Kaye, and Ian Rogers.

That seems like a lot more work than necessary, so I'm going to try to 
trim the process quite a lot and knock out the job with whatever the 
minimum necessary is.
