[Playlist] More Version 1 document proofreading

I would suggest a new section in the spec, or a footnote, that describes
the revision history since the spec has been frozen (dates and summary
of changes), just to make clear that there have in fact been changes to
the document itself, but none of them affect implementation of the spec.
