[Playlist] Defects in XSPF files out in the wild

I was thinking about repairing mechanisms for
defects found in XSPF files out in the wild.

What defects did we find? What I currently
remember:

 * XML errors (like unencoded ampersands)
 * <artist> instead of <creator>
 * Filenames instead of URIs in <location>
 * Wrong namespace URI
 * Non-XSPF elements outside of <extension> tags
 * Missing attributes like "rel" for <link>

Anything else?
